Repair Authorization. By checking the box below and typing my name, I authorize Cornerstone Auto Pros to perform hail damage repairs on the vehicle described above, including the inspection and teardown reasonably necessary to identify the full extent of the hail damage. I understand additional damage may be found once work begins, and I authorize the shop to document it and coordinate a supplement with my insurance carrier before that work is performed.

I authorize my insurance carrier to include Cornerstone Auto Pros on any payment for these repairs and direct that it be applied to the cost of the repair. I understand I am responsible for my insurance deductible as stated in my policy and for any non-covered charges I approve in writing.

Most hail repairs are completed within a few days to a few weeks after the insurance estimate is approved, depending on damage severity, parts availability, and scheduling. Cornerstone Auto Pros performs the right repair using high-quality parts and skilled technicians, and works collaboratively with my insurer and, when needed, specialty paint and body partners — so I get the best long-term result.

Cornerstone Auto Pros provides a free assessment of hail damage. This authorization governs the repair work once the insurance estimate is approved.

Limited Power of Attorney — Insurance Payments. I appoint Cornerstone Auto Pros as my limited attorney-in-fact for the sole purpose of this claim (claim number shown above), authorizing the shop to receive, endorse, and sign on my behalf any insurance check, draft, or payment issued in connection with this claim, and to apply those funds to the cost of the repairs described in this agreement.

This limited authority applies only to payments concerning this claim and this repair — it does not authorize any other act. I remain responsible for my insurance deductible as stated in my policy. This authority is effective until the repair is completed and paid.
